Each survey page created by IntelliSurvey allows survey takers the opportunity to provide feedback:

Clicking the button launches a form, with two fields:

The first field is a closed-end drop down, with the following options:

• Good news
• A suggestion for improvement
• Survey seems stuck
• Typo or incorrect text
• Available answers are insufficient
• Question not relevant to me
• Other survey problem.

The second is an open-end text entry field.

These two fields can help researchers understand what issues may exist in a survey by tying specific feedback to the page upon which it was submitted.

Override feedback options

Feedback options can be customized by including a custom Survey Message:

Disable feedback

The feedback box shows by default. To disable the feedback box, include the feedback tag (which accepts 'y' (yes) and 'n' (no)) on the app config widget, and set the value to 'n'.
